Memorial Hermann Health System in Houston is launching new AI-enabled remote care services through a partnership with Cadence to support patients with chronic conditions.
The health system plans to roll out remote patient monitoring and advanced primary care management programs that provide continuous support between in-person visits, particularly for older adults and those with conditions such as hypertension, congestive heart failure and Type 2 diabetes.
Patients enrolled in the program will receive 24/7 at-home support while care teams track real-time data, including blood pressure, heart rate, weight and glucose levels, to identify potential issues earlier and adjust care plans, according to an April 30 news release.
The platform combines AI with a team-based clinical model that extends primary care services outside traditional settings. Memorial Hermann said the advanced primary care management component will focus on patients 65 and older, with the goal of closing gaps between visits through ongoing clinical guidance.
